Reforms to Guardianship and Administration Act 1995 (the Act) on 1 September 2024 – new Health Practitioner Report forms

The Tribunal has today released the new Health Practitioner Report forms that will be required under the new provisions of the Act that will commence on 1 September 2024 for applications for:

The Tribunal has released these forms early to assist applicants in submitting health practitioner reports in accordance with the Act from 1 September onwards. If you believe you will be lodging an application for guardianship, administration, or both, or reviews of such orders, then please use the new forms.

Please note there are transitional provisions under Act (Section 90BA) that will allow the Tribunal to continue to accept applications that are not in accordance with the new provisions of the Act up until 1 November 2024.

However, applicants are encouraged to use the new forms.

Please note, from 1 November 2024 the new forms must be used.

The Tribunal will also release new application forms, fact sheets and information closer to 1 September 2024 to assist all parties and applicants in meeting the new statutory requirements. If you intend lodging an application before 1 September 2024, you must continue to use the current application forms.
